Staged bilateral DIEP flap reconstruction for nipple-sparing mastectomy

Patient underwent two-stage breast reconstruction for bilateral nipple-sparing mastectomy (through infra-mammary fold incisions). Photos show pre-mastectomy appearance, the middle stage after radiation with bilateral tissue expander in place, and after delayed DIEP flap reconstruction

Unilateral DIEP flap reconstruction

Before (Figures 1 & 2) and After (Figures 3 & 4). Patient with unilateral mastectomy after multiple lumpectomies for breast cancer. She desired natural ptosis (natural sagging or hang) to match her right breast. She did not want the right breast to be lifted and preferred to minimize her interventions. She did not want nipple tattooing or staged nipple areolar reconstruction.
